fbx_FormBuilder is an application I originally wrote several years ago so that our organization's end users could create their own on-line forms and manage the results, without IT Department intervention.

It's not especially fancy, but it's been used extensively by people in our organization, so I thought others might like to look at or use the code.

Earlier this year, it was updated to use the Fusebox 5 framework.
